Who is responsible for your data

Fati Soltani — Mondhygiënistenpraktijk, Sloterweg 1160, 1066 CV Amsterdam, is the data controller for the personal data processed through https://www.mondhygienisteamsterdam.nl. You can reach us at info@mondhygienisteamsterdam.nl.

What we collect and why

We only collect what we need in order to treat you well and to run the practice.

  • When you book online: your full name, date of birth, home address, email address, phone number, the treatment you selected, the date and time, and any note you add. Name, date of birth and address are needed for your patient record and to invoice your health insurer; the rest is used to reserve the appointment and contact you about it.
  • When you register as a new patient through the website: your name, date of birth, gender, contact details, address, the reason for your enquiry, how and when you prefer to be contacted, and any note you add. We deliberately do not ask for medical information through the website — your medical history is taken in the practice at your first appointment and kept in your patient record, not on this site.
  • When you send a contact message: your name, email address, phone number if you give it, and your message. This is kept so that we can answer you and, if you become a patient, see what was discussed. If you arrived through an online advertisement, we also store the advertising click identifier so that we can see which advertisements lead to appointments.
  • Technical data your browser sends automatically, such as your IP address, used to serve the site and keep it secure.

Legal basis

Appointment and registration data are processed to perform the agreement between you and the practice, or to take steps at your request before entering into one. Health data is not collected through this website; in the practice it is processed for the purpose of healthcare, under Article 9(2)(h) of the GDPR and the Dutch Medical Treatment Agreement Act (WGBO). Advertising click identifiers are processed on the basis of our legitimate interest in measuring which advertisements work. Where we ask for your consent, you may withdraw it at any time.

Who else sees it

Your appointment is added to the practice calendar, which is hosted by Google. The website is hosted by Vercel; the database is hosted by Neon in Frankfurt, Germany (EU). Confirmation and notification emails are sent through Resend. These providers process data on our instructions under a processing agreement. We do not sell your data and we do not use it for advertising.

How long we keep it

Medical records are kept for twenty years from the last treatment, as the WGBO requires. Enquiries that do not lead to treatment are deleted within one year.

Your rights

You may ask to see your data, to correct it, to have it deleted where the law allows, to restrict or object to processing, and to receive a copy. Write to us at the email address above and we will respond within one month. If you are not satisfied, you may complain to the Dutch Data Protection Authority (Autoriteit Persoonsgegevens).
